Premature Ejaculation (PE), experienced by one in three men at some point in their lives, carries the distinction of being the most common sexual dysfunction. Defined as the man ejaculating outside of his control and volition before sexual intercourse has even occurred, immediately after intercourse, or during masturbation, premature ejaculation can continue throughout life or emerge later.
Although it is generally thought that premature ejaculation is a problem only for the man, the woman is also negatively affected by this sexual dysfunction, and ultimately, the relationship between the couple can be damaged.
Performance anxiety may develop in men experiencing premature ejaculation problems, and premature ejaculation and erectile dysfunction can be triggered due to performance anxiety. Premature ejaculation, which leads men to feel sexually inadequate and women unable to reach sexual satisfaction, can also harm the bond between couples. Premature ejaculation, which should be evaluated as a health problem concerning the individual, the family, and society, can be managed thanks to personalized treatment processes.

Premature Ejaculation Treatment Methods
Premature ejaculation problems can arise due to both physical and psychological reasons such as environmental factors, lifestyle, stress, performance anxiety, prostate infections (prostatitis), urinary tract infections, hormonal problems, and faulty circumcision surgery.
When planning premature ejaculation treatment, the factors causing PE must be analyzed correctly. This sexual dysfunction, which can sometimes be managed with lifestyle changes, may sometimes require the simultaneous application of different treatment methods.
1. Observation Therapy (Watchful Waiting) for PE
During observation therapy, which is based on changing lifestyle dynamics such as stress, malnutrition, excess weight, alcohol, and smoking that can cause premature ejaculation, no medical treatment is applied to patients. During observation therapy, the patient is asked to take the necessary actions, and the change is closely monitored. If observation therapy alone is not sufficient, a transition to medical treatment for premature ejaculation is made.
2. Medical Treatment for PE
For patients who do not achieve results with observation therapy, a transition to medical treatment is made. How long medical treatments will be applied may vary depending on the patient. The aim of medical treatments applied for premature ejaculation is the suppression of the sympathetic nervous system that controls ejaculation.
Drugs containing Dapoxetine, the only active ingredient approved for use in the treatment of premature ejaculation, can prolong the ejaculation process by 5-7 times if used at a dose determined specifically for the person for at least six months. Within the scope of premature ejaculation treatment, the use of desensitizing creams, gels, or sprays that reduce penile sensitivity may also be recommended.
3. Exercise and Habit Change in PE Treatment
Exercises that delay the ejaculation process may also need to be performed during premature ejaculation treatment. Ejaculation time can be delayed thanks to pelvic floor exercises (Kegel exercises) that ensure the control of pelvic muscles and the stop-start technique.
Premature ejaculation treatments are not limited to what I have listed; studies on new treatment approaches are ongoing. Finally, the treatment of premature ejaculation with Botox is being studied. It is known that results are being obtained indicating that Botox, which has been used for a long time in the treatment of strabismus, migraine, and sweating, especially for the removal of skin wrinkles, will delay premature ejaculation by preventing rhythmic contractions.
4. Delaying Premature Ejaculation is Possible with Glans Filler Application
The glans penis (head of the penis) has an anatomy with high nerve density. Men experiencing premature ejaculation problems due to penile hypersensitivity can control premature ejaculation and step into a more satisfying sexual life thanks to the glans filler application.
During the procedure performed in a clinical setting (office-based), the application area is numbed with local anesthesia, and then dermal filler materials are injected into the head part of the penis called the glans. Dermal fillers containing Hyaluronic Acid create a barrier between the nerves in the glans penis and the skin. This situation makes it possible to achieve a significant improvement in premature ejaculation. The ejaculation problem regresses to a great extent within 1 week after the application.
Conclusion
It should not be ignored that premature ejaculation is a health problem whose negative effects can be significantly reduced and delayed. When the causes are correctly identified and personalized treatments are planned, psychological stress caused by premature ejaculation can be avoided, and couples can have the chance to leave the problems in their relationships behind.
